1. Information Forma Collects

The information Forma collects depends on how a visitor uses the website and which information the visitor chooses to provide. Forma may collect information submitted through forms, assessments, Typeform, emails, or other communications, including name, work email, telephone number, company information, business role, service industry, location, approximate company size or revenue range, assessment responses, scheduling preferences, messages, communication preferences, and other information voluntarily provided. Visitors should not submit customer records, passwords, full payment-card information, Social Security numbers, protected medical information, confidential employee records, or other sensitive information through general website forms.

Depending on the active configuration, Forma and its service providers may automatically collect IP address, browser and device information, operating system, approximate IP-derived location, referring page, pages viewed, links selected, visit timing, time spent, form interactions, performance information, and cookie identifiers. The exact information collected automatically depends on the analytics, hosting, form, and website tools active at the time of the visit.

2. How Information Is Collected

Information may be collected when a visitor visits the website, interacts with cookies, completes the Revenue Leak Check, submits a contact form, subscribes to communications, replies to an email, communicates with Forma, becomes a client, is referred to Forma, or interacts with an embedded third-party service. Submitting information through an embedded form may provide information to both Forma and the company operating that tool.

3. How Forma Uses Information

Forma may use information to respond to inquiries, provide requested assessment results, evaluate service fit, prepare proposals or agreements, provide and improve services, operate and maintain the website, understand usage and performance, diagnose technical problems, prevent fraud or security incidents, maintain records, comply with legal obligations, establish or defend legal claims, send permitted marketing communications, and honor unsubscribe or privacy requests. Forma will not use information submitted for one purpose in a materially different way without an appropriate basis or additional notice when required.

Current Website Tools: Framer provides the website platform and Framer Analytics; Typeform provides embedded assessment forms; Google Analytics provides website analytics. These providers may receive technical information, interaction data, and the information necessary to operate their respective services under their own terms and privacy practices. Visitors may manage certain technologies through browser settings. Forma does not currently provide a Forma cookie-preference center.

5. How Information May Be Disclosed

Forma may disclose information only for legitimate business, operational, contractual, security, or legal purposes. Information may be provided to service providers supporting website hosting, form processing, assessment delivery, email delivery, customer relationship management, analytics, file storage, security, spam prevention, payment processing, or professional services. Information may also be disclosed to professional advisors, when needed for legal and safety reasons, in connection with a business transfer, or with a person’s direction or consent.

Sale or Sharing of Personal Information: Forma does not sell personal information for money and does not knowingly share personal information for cross-context behavioral advertising based on the current verified configuration.

6. Third-Party Services

Some website features are provided through embedded or linked third-party services. Framer is the verified website platform and publishing provider. Typeform is the verified assessment-platform provider. Google Analytics and Framer Analytics are verified analytics providers. When a visitor interacts with these services, they may collect information under their own privacy policies and terms. Links to third-party privacy policies are provided for convenience; Forma does not control how independent third parties maintain or change their privacy practices.
